Update mode and connection errors
Every connection status code maps to one remedy.
Connections report a status_code on /install/get and in connection.status_changed webhooks. When action_required is true, create a link token with mode: "update" and the install’s access_token, and open Link again.

| APPROVAL_PENDING | Waiting for the carrier's manual review | None; Relay notifies on change |
| VERIFICATION_REQUIRED | The agency must complete roster OTP or SSO | Launch Link in update mode |
| AGENCY_ADMIN_PENDING | A join request awaits the agency admin | None (agency side) |
| CODE_REMOVED | The carrier removed or terminated a code | Update mode (fix code) or remove |
| CARRIER_PAUSED | The carrier paused delivery, or Relay suspended it | None |
| DENIED | The carrier denied the request (reason included) | Update mode to correct the code |
